For (1), this warning message is normal. Your data only extends from 1000 to 200mb, yet
you are requesting the display extend to 150mb and below to 1050mb. It's just a warning.
For (2) You would have to add an extra level to the output in the file. Before writing 1000mb
data, you should write your surface pressure data for temperature, etc. And then add
the surface pressure as the first level in the ZDEF line.
